The Health Checks are rolling in and within two months we have reached our first century!
There has been a steady increase in the number of Health Checks that have been received by the Children & Young People’s Health Partnership (CYPHP). But, we would like more!
The Health Checks are part of a population-based intervention to help families understand more about their child’s health and wellbeing, and help health professionals to tailor care more closely to the child’s needs. Health Support Packs support parents, carers, and children to increase their knowledge about asthma, constipation, eczema, and/or epilepsy, and about general wellbeing too.
Health Support Packs provide information about condition management, top tips about family wellbeing, and useful community resources.
CYPHP’s Health Checks and Health Support Packs are an important part of our approach to early intervention, prevention, and health promotion. They help us provide child-centred care, and to improve services for families who may face challenges in engaging with care.
Information from CYPHP’s Health Check forms part of GP care records, so health professionals involved with a child or young person’s care have more holistic information to help them deliver better care to their patients.
CYPHP Health Checks have been distributed to all GP practices, and we’ve been asked for advice about how best to promote them and distribute them to families. A member of the CYPHP team can come to one of your GP Practice management or clinical team meetings to talk about the Health Checks.
