The CYPHP Health Check is currently suspended.

This is because the CYPHP Health Team is working to support the NHS to manage the coronavirus and essential services. If your child has been receiving services from the CYPHP Health Team, you would have received advice and information on how to continue to manage your child's condition.

If you do become concerned about your child's health, please get in touch with your local GP or contact 111 for advice and guidance. 


The Health Check was offered to children and young people registered with a GP Practice in the London boroughs of Lambeth and Southwark. As a parent/carer, please bear with us until we can once again invite parents/carers to complete the Health Check online.

Once completed, CYPHP would send you your child's free Health Support Pack. Each pack includes:

  • Practical information about your child's condition.
  • Top tips to manage their triggers.
  • Useful information about their wellbeing.
  • Helpful community resources in your area.

Who could peviously receive the Health Check and Health Support Pack?

Parents/carers were able to complete the Health Check for their child if their child is:

  • 0-15 years old; and
  • diagnosed with asthma, constipation, or eczema; and
  • registered with a Lambeth or Southwark GP Practice

Can I complete a Health Check now?

To support the NHS to manage the coronavirus and essential services, the Health Check is currently not available.

We will make it available online as soon as we are able. 


What would I be asked in the Health Check?

The Health Check has three sections. It would ask you about your child's:

  • asthma, constipation, or eczema symptoms
  • their general emotional and behavioural wellbeing
  • some family background information

What would happen to the information in the Health Check?

The Health Check summary would be added to your child's GP record. It would give your child's health team a better understanding of your child's health and general wellbeing.

You could also request a copy of your child's results and a Health Support Pack.

Your child's information would not be used for any other purpose without your consent.
