What is the CYPHP Health team?

The CYPHP Health team works in the community alongside other child healthcare professionals in Lambeth and Southwark. Our team includes Community Nurse Specialists and Mental Health clinicians at the front line of care, backed up by Paediatricians, Child and Adolescent Psychiatrist, and GP. Our team sees children and young people that:

  • Have a diagnosis of asthma, constipation, eczema, or epilepsy.
  • Are between the ages of 0-16 years old.
  • Registered with a participating GP practice in Southwark and Lambeth.

 

What do we do?

Our specialist CYPHP Health Team look after the whole child, not just their condition. We understand that emotional and behavioural issues can be common in children and young people who have other health conditions, and that families often have other difficulties which can also influence their child’s health and wellbeing.

We do our best to arrange clinic appointments close to home, keeping in touch with other healthcare professionals involved in the child's care. So we aim to support children and families holistically, and make a real difference to their lives.

We’ve heard about CYPHP care from families who had just experienced the service – all were encouragingly positive!  25 families who received CYPHP care between January 2019 and May 2019 said that they either agreed or strongly agreed that they:
