Last month CYPHP celebrated its one Year Anniversary of the Health Check. The past year has been exciting to say the least, with many positive outcomes and intriguing insights (to find out more click on this link).
By working in partnership with local GP surgeries and other community healthcare professionals, (for example using the GP call and re-call system for proactively engaging with patients to improve early intervention and prevention), and by hosting and attending partner and community events, the CYPHP Health Check has been shared with many families across Lambeth and Southwark. To date, more than 2,600 Health Checks have been completed.
To help get the message out to more families, and as part of the one Year Anniversary, two videos were created to make it easier for families and professionals to know what the Health Check is, and what the benefits are of completing it. You can view them here:
