With the roll out of call-recall in Lambeth and Southwark GP practices, families have been encouraged to complete the Health Check via SMS messages, letters, and emails. Call-recall is an important part of our approach to population health. It’s a way for us to be proactive and reach children with ongoing conditions, to help them stay well.
We are excited to see that call-recall has positively contributed to improved access to care, reducing inequities. This is reflected in a rise in Health Check completion rates; currently more than 1700 parents/carers in Lambeth and Southwark have completed the Health Check.
The CYPHP Health Support Pack is sent to each family after completing the Health Check. Each pack is filled with practical information about their child’s condition, ways to identify and manage triggers, and useful community resources. Health Packs are all about health promotion and supported self-management.
To ensure that this great resource is available to all families, we are asking healthcare and non-healthcare professionals to please share the website link below with eligible families. They include parents/carers of children:
- aged 0-15 years
- with either asthma, constipation, epilepsy, or eczema
- and registered with a Lambeth or Southwark GP
