CYPHP clinics deliver more joined up care than ever.
Throughout 2018, CYPHP clinics have continued to improve. Patch Paediatricians (Consultant Paediatricians from Evelina London and King’s College Hospital) are forging strong links with local GPs, helping improve local care and support.
CYPHP is proud to announce that 10 Patch Paediatricians are regularly holding joint clinics with GPs in Lambeth and Southwark practices. Together, they are seeing children with any problem who would otherwise have been referred to General Paediatric Outpatients. The result is an inspiring list of benefits for patients and professionals:
- Children are able to receive specialist care in a more familiar environment, and closer to home.
- The average waiting time is greatly reduced - average of 18 days compared with two to three months for a hospital appointment.
- The healthcare team has access to Primary Care records, providing valuable continuity and insights into a child’s care over time.
- Clinicians feel they are able to offer better and more holistic care, working together on care plans and co-ordinating follow up, building strong interprofessional teams.
Feedback from families continues to be very positive, with 90% saying they would recommend the service to a friend.
