The Children & Young People’s Health Partnership (CYPHP) started distributing free Health Checks at the end of March to all GP practices in Lambeth and Southwark. The Health Checks with Health Support Packs are designed to help tailor care to the whole child in the context of their family and community, and to provide parents/carers with health information, support for general wellbeing, top tips on family health and wellbeing, and useful community resources.
Our aim in CYPHP is to work with parents/carers and other health professionals so that children can stay as healthy as possible, get better quickly when they’re ill, and to live well if they have a long-term condition. Health checks and support packs are part of our preventative and public health care, and along with other parts of the service, should help to improve health and decrease parents/carers reliance on A&E, GP, and hospital outpatients care.
Each pack sent to GP surgeries contains materials to support parents/carers to complete a Health Check if their child is aged 0 – 15 years old and has either asthma, constipation, eczema, and/or epilepsy. Parents and carers can either complete and hand the Health Check to the GP reception who will post it to us, or can use a CYPHP addressed free-post envelope if it is more convenient to complete the Health Check at home.
Free Health Support Packs will be sent out to everyone who completes a Health Check, and will soon be available on-line via our CYPHP patient portal.
