This week we celebrated a milestone for the CYPHP programme. 100% of practices in the intervention arm of our evaluation have now agreed to offer CYPHP clinical services to their patients and also to share data through EMIS (the clinical information system used in GP practices). This will mean that more patients than ever can benefit from interventions like inreach and specialist nursing for long term conditions, and that professionals from different organisations can work together more effectively than ever before as they have access to essential patient information from general practice.
This achievement is the outcome of a lot of collaborative work across the partnership, especially from our colleagues in general practice. Melbourne House Surgery was the practice that took us to the 100% mark - their practice manager, Golam Morsed, is pictured with Ryan Little and Julien Hersh from the CYPHP team as they marked the moment.
